MELANOMA COROIDEO: RETINOGRAFÍA E IMAGEN RADIOLÓGICA
Aunque la mayoría de los melanomas derivan de losmelanocitos de la piel, la segunda localización más frecuente del melanomaprimario es el ojo. Constituye además la neoplasia maligna intraocular másfrecuente en los adultos. Aproximadamente el 85% de los melanomas oculares selocalizan en la úvea y el 4,8% en la conjuntiva . La exposición a radiación UV-B y la falta de uso degafas de sol se han asociado con riesgo de melanoma uveal. Otros factores deriesgo relacionados son pigmentación clara, melanocitosis ocular, síndrome denevus displásico y presencia de mutaciones genéticas. En lo que se refiere a los síntomas puede aparecerdisminución de agudeza visual, alteración del campo visual, fotopsias,miodesospsias, dolor o visualización del tumor. El 30% de los casos sonhallazgos casuales en pacientes asintomáticos.El diagnóstico del melanoma uveal con frecuencia puederealizarse por biomicroscopía en casos de afectación iridiana o mediante examende fondo de ojo en aquellos que afectan a la úvea posterior, sin necesidad deconfirmación histopatológica en la mayoría de los casos. Para corroborar eldiagnóstico o caracterizar mejor la enfermedad son útiles otras pruebas deimagen como la angiografía fluoresceínica, la ecografía o la resonanciamagnética nuclear.Debido a suaccesibilidad, inocuidad y bajo coste, la ecografía sigue siendo la prueba deprimera aproximación diagnóstica, siendo la RM la técnica de elección para eldiagnóstico diferencial, extensión, asociación de complicaciones y seguimiento.El hallazgo típico es una masa sólida lobulada, sésil o en forma decúpula, localizada en la cámaraposterior. Otras técnicas de imagen para su diagnóstico son la RM y latomografía axial computarizada.
