Laneumonía es una entidad que consiste en la infección aguda del parénquimapulmonar que genera manifestaciones sistémicas, síntomas respiratorios agudos yse acompaña de infiltrados parenquimatosos en la radiografía de tórax. Pese alos avances médicos de las últimas décadas, sigue siendo una causa importantede morbilidad y mortalidad en la población general. La actitud terapéutica anteun paciente con neumonía debe incluir la necesaria cobertura antibiótica de losmicroorganismos causales y el tratamiento de soporte de la alteraciónfisiopatológica que produce la infección pulmonar. La evaluación inicial de lagravedad del paciente con neumonía comunitaria es clave para establecer eltratamiento y la ubicación más apropiada para su cuidado. Enlos pacientes con neumonía grave adquirida en la comunidad que presentan insuficienciarespiratoria aguda, el uso de ventilación no invasiva como técnica de soporterespiratorio es controvertido. En la actualidad se disponen de pocos estudios bien diseñados en los que se analice laeficacia de la técnica en estos pacientes. La mayoría de estudios ya seaobservacionales o controlados valoran el soporte ventilatorio no invasivo engrupos heterogéneos de pacientes, mezclando diferentes etiologías del fracasorespiratorio e incluso a veces, pacientes con fracaso hipoxémico ehipercápnico. Independientemente del tipo de estudio, lo que la literaturademuestra es que el paciente con neumonía presenta un riesgo muy elevado defracaso de la ventilación no invasiva, muy por encima de los pacientes con EPOCo edema agudo de pulmón cardiogénico. Según algunos estudios, los pacientes conneumonía que presentan patología respiratoria o cardíaca previa responden mejor a la ventilación no invasivaque aquellos con insuficiencia respiratoria ?de novo?.En este libro se analiza de forma exhaustivo laliteratura científica, pudiendo concluirse que la aplicación de ventilación no invasiva puede serefectiva en los pacientes con neumonía grave adquirida en la comunidad, aunquelos mejores resultados obtenidos deben de esperarse en los pacientes con menorafectación clínica, gasométrica y radiológica. Además, esta técnica es seguraya que las series publicadas no presentan un número excesivo de complicacionesque desaconseje la realización de la técnica.
